Utah Governor’s Office of Economic Development

All contestants are required to have a current (less than 6 months old) HIV test and Hepatitis B & C test results (less than 1 year old) prior to their bout.  All contestants are required to submit a completed licensing physical with their initial or license renewal.  Contestants 36 years of age and older are required to have an EKG on file that is less than 2 years old and reviewed by a physician.

The fee for unarmed combat licenses is $30, with the exception of the promoter license which is $125.  An unarmed combat license is valid for 1 year from the date of issuance.  Contestants are also required to have a current MMA ID or Federal Boxing ID.  The fee for a new or replacement ID card is $10.
